Almost every peptide is given by injection. That isn't tradition or laziness — it's biology. Here's the actual obstacle, and why it's so hard to get around.
The problem in one sentence
Your digestive system is extremely good at destroying peptides, because peptides are made of the same material as food.
Why swallowing fails
A peptide is a short chain of amino acids. Protein in your lunch is a long chain of the same building blocks. Your stomach cannot tell the difference — and it isn't supposed to.
Two things go to work immediately:
- Stomach acid, which is strongly acidic and begins breaking chains apart.
- Enzymes, which act like molecular scissors, cutting chains into fragments small enough to absorb.
By the time anything reaches the intestine, a swallowed peptide has largely been dismantled. It hasn't been absorbed — it's been eaten.
The second obstacle
Even a peptide that survives digestion faces another problem: peptides are relatively large and water-loving molecules, and the intestinal wall does not let such molecules through easily. Surviving the acid isn't enough; it still has to get across.
Why injection works
An injection simply skips the entire problem by placing the peptide past the digestive tract altogether. It's effective — and it's why needles have been the default for decades.
The sublingual idea
There's another route: the tissue under your tongue and inside your cheek — the sublingual and buccal mucosa. That tissue is thin and rich in small blood vessels, and importantly, anything absorbed there enters circulation without first passing through the stomach.
This is why dissolvable films are an area of active research. The concept is straightforward. The execution is not.
Why it's genuinely hard
Each peptide is a different molecule with its own size, stability and chemistry. A film that works for one may fail for another. Real formulation work has to address:
- Protecting the peptide inside the film
- How fast the film dissolves — too fast or too slow both cause problems
- Moisture, which peptides are sensitive to
- How much actually crosses the mucosa (bioavailability)
- Stability on a shelf, not just on day one
- Taste, since it's in someone's mouth
The honest state of play
Sublingual delivery is not a solved problem. Absorption across the oral mucosa is typically lower than injection, and how much lower depends on the specific peptide. Anyone claiming a universal needle-free solution for all peptides is overstating things.
